WomensLink Follow-up Interview
This interview was developed by The Measurement Group and
PROTOTYPES and was used to measure client outcomes related to health and
psychosocial functioning, service linkages, and overall program feedback.
Visiting
Nurse Association of Los Angeles; Phone Interviews Conducted by TMG
This form was developed to use as a telephone structured telephone interview to evaluate
outcomes for patients at the VNA-LA. The structured interview includes measures of health
status, health-related quality of life, and emotional readiness to accept the patients
stage of disease.
National
AIDS Update Conference Health Initiatives for Youth Workshop Evaluation Form
This form was developed by The Measurement Group for Health Initiatives for Youth
(HIFY) to evaluate the youth component of the National AIDS Update Conference workshops.
Key Informant Interview for the
Comprehensive Healthcare Work Group
This semi-structured interview was developed for data collection by the Comprehensive
Healthcare Work Group. The purpose of the interview is to elicit feedback from
stakeholders in each of three comprehensive healthcare service delivery programs for
persons living with HIV/AIDS. The interview is designed to obtain information about the
progress of these projects in areas such as patient recruitment and retention,
accessibility, quality of care, education and training, consumer involvement, development
of service networks, and information dissemination. The instrument yields quantitative as
well as qualitative data and may be administered on the telephone or face-to-face.

DC Collaborative for Women with HIV/AIDS Needs Assessment Instruments
This set of evaluation instruments was developed by The Measurement Group
to conduct need assessment among healthcare and social service agencies in
the greater Washington D.C. area.
Module P1: Adherence to Protease Inhibitor
Therapy Interview
This interview template was developed by The Measurement Group and grantees,
especially Dr. John Bartlett and Lois Eldred of the Johns Hopkins University School of
Medicine and is used to gauge a patient's overall adherence to their Protease Inhibitor
therapy. This interview is presented as a sample of a feedback form which can be used to
assess how well a patient is adhering to their drug therapy.
Structured Telephone Interview for University of
Texas Collaborators
This telephone interview template was developed by The Measurement Group and is
used to assess a collaborating agency's involvement with and perceptions of the University
of Texas Health Science Center's Project SALUD. This interview is presented as a sample of
a feedback form which can be used to assess how well collaborating agencies work with one
another.
University of Nevada Project: Client Telephone
Interview
This telephone interview template was developed by The Measurement Group and is
used by the University of Nevada School of Medicine to gather information about the
experience and services received by clients of their HIV nutrition clinic. This interview
is presented as a sample of a client feedback interview to give information about an HIV
clinic.
Training Outcomes
Interview for the Training Work Group
This semi-structured interview was developed by The Measurement Group and
Training Work Group. This interview was developed for the Training Outcomes Study to
obtain information about the effectiveness of the trainings on systems change and changes
in patient care.
